Remember to include your registration number in the Google form when completing the application. Your registration number will be cross-checked during the selection process and document verification.

Selected candidates should bring the following documents at the time of joining:
- Police Verification certificate from their local police station: and
- Conduct/Character certificate from the Head of a previous educational institution.
- A Physical Fitness Certificate from a Civil Assistant Surgeon
- SSC Certificate
- ITI certificate
- Caste/PWD Certificate. Expired OBC certificates will not be considered.
- Copy of bank passbook.
- Adhaar
- One Recent passport-size photograph
